Survival:

- Updated browsing layout (moved map name to bossbar, added difficulty display) (thanks @MisterMel)
- Map difficulty is now displayed with color in the bossbar (thanks @MisterMel)
- Fixed errors related to /fp maps (thanks @MisterMel)
- Players now receive a title message when entering play, browse or build mode (thanks @MisterMel)
- Fixed issue with not receiving creative mode when creating new map
